Formstack Forms: Saving File Uploads to SharePoint, Teams, or OneDrive

Summary

IT&A can connect your Formstack form to a SharePoint site, a Microsoft Teams channel, or a OneDrive folder. Every file uploaded through the form is then saved there automatically, with its original file name and extension. The automation handles one or many files per submission.

Audience

This article is intended for faculty and staff who own or manage a Formstack form.

Platform

Formstack Forms, Microsoft SharePoint, Microsoft Teams, Microsoft OneDrive, and Microsoft Power Automate

Overview

There are two methods for saving uploads to OneDrive or to a shared SharePoint library or Teams channel.

  • Using Formstack's built-in OneDrive integration that saves uploads to one person's OneDrive. After setup, each time someone submits your form, the file will be saved to your chosen folder.
  • Using a Microsoft Power Automate flow to save uploads to one person's OneDrive or to a shared SharePoint library or Teams channel. After setup, each time someone submits your form, the file will be saved to your chosen folder, named <date>_<submission ID>_<original file name>. For example: 2026-09-28_1501655623_travel_waiver.pdf. The submission ID in the file name matches the Unique ID on the submission in Formstack, so every file can be traced back to its entry.

Using Formstack's Built-In OneDrive Integration

Formstack's built-in OneDrive integration saves uploads to one person's OneDrive. After setup, each time someone submits your form, the file will be saved to your chosen folder.

  1. Start by creating a new form or opening an existing one in Formstack Forms. If you plan to send uploaded documents to OneDrive, confirm that the form includes at least one File Upload field. Formstack supports file uploads of up to 2 GB total per submission.
  2. Go to the form’s integration settings by selecting your form's Settings > Integrations.
  3. Click Add next to Microsoft OneDrive.
  4. Select Log in to Microsoft OneDrive, and then sign in with the Microsoft account you want to use for file storage.
  5. Approve the requested permissions to authorize the connection between Formstack and OneDrive.
  6. Configure the integration settings for your form. Choose the OneDrive location or storage behavior you want Formstack to use for uploaded files.
  7. Turn on the Microsoft OneDrive integration.
  8. Submit a test entry with a sample file. Then confirm that the file appears in OneDrive and that the submission record in Formstack includes it.

Using Power Automate

Complete these three steps first so IT&A can finish the setup in a single pass.

Prepare your form

  1. Create or open your form in Formstack Forms.
  2. Add at least one File Upload field. To accept several files in one field, open the field and check Allow multiple files.
  3. Note the form's name and its URL from the browser address bar, for example https://www.formstack.com/admin/form/builder/1234567.

Choose and share the destination folder

Pick one of the destinations below. Then give the IT Forms & Survey Support account Edit access to the folder, so the flow can save files there.

Destination

Best for

What to send IT&A

Teams channel

A department or committee that works in Teams

Team name, channel name, and the folder inside the channel (for example Applications 2026)

SharePoint site

Department sites and document libraries

Site URL (for example https://ithacaedu.sharepoint.com/sites/YourSite), library name, and folder

Your OneDrive

Forms only you review

The folder path in your OneDrive (for example Formstack Uploads)

Request the automation

Submit a Web Forms request in the IT Service Portal and include:

  • Form name and URL
  • Name of the File Upload field (if the form has more than one, list each and where its files should go)
  • Destination: Teams channel, SharePoint site, or OneDrive, plus the exact folder

Best Practices

  • Use a dedicated folder. Send uploads to a folder used only for this form, for example Formstack Uploads/Scholarship Applications. That keeps access control, retention reviews, and cleanup simple.
  • Prefer a shared location for team processes. If several people review the files, use a Teams channel or SharePoint site rather than a personal OneDrive, so access doesn't depend on one person's account.
  • Limit who can see the folder. Everyone with access to the destination folder can open every uploaded file. Check the folder's sharing before go-live, especially for sensitive documents.
