Microsoft Forms facilitates the creation and distribution of surveys, quizzes, and polls. It offers a user-friendly interface that enables both individuals and organizations to gather data efficiently.

Creating a Form or Quiz

  1. Go to https://forms.office.com/
  2. Select the New Quiz button or the New Form button to create a new personal form. For shared forms, first, choose a Microsoft 365 group or team under My Groups, and then select the New Group Quiz button or New Group Form button to create a new shared form.
  3. Enter a title for your form and an optional description.
  4. Select Add New to add a form field. You can choose to add Choice, Text, Rating, or Date questions. Select the drop-down list to see more question types, such as Ranking, Likert, File upload, Net Promoter Score, and Section.
    • Form fields can be made required, include branching, and have subtitles, among other options.
  5. Select Preview at the top of the design window to see how your form will look on a computer or mobile device.

Customizing

When building your form, the form can be customized with a unique background image or color. Background music should not be added to a form.

  1. Within your form, click Style at the top of the design window.

Settings

To adjust settings in your Microsoft Form, first click the ellipsis (…) located at the top of the design window, then select "Settings". The Settings section offers a range of options to tailor your form or quiz:

  • Define the audience who can respond to the form or quiz.
  • Control the form's availability for receiving responses.
  • Specify start and end dates for response collection.
  • Implement a time limit for completing the form or quiz.
  • Randomize the order of questions for each respondent.
  • Choose to hide question numbers from respondents.
  • Include a progress bar to indicate completion status.
  • Opt to remove the "Submit Another Response" button.
  • Customize the message displayed upon submission.
  • Allow respondents to save their progress and edit responses later.
  • Add or personalize receipts confirming response submission.

Sharing the Form or Quiz

To initiate response collection for your form, navigate to the top right corner and select Collect Responses. You'll have several options for determining who can respond:

  • "Anyone can respond" allows for responses from both inside and outside your organization.
  • "Only people in my organization can respond" restricts responses to those signed in with a work or school account.
  • "Specific people in my organization can respond" limits access to designated individuals or groups within your organization.

Next, choose your preferred method for requesting responses:

  • Via link: Click the Link button, then the Copy button next to the displayed web address. Share this unique link with your audience, allowing them to access the form or quiz.
  • Via QR code: Select the QR code button, followed by the Download button to obtain the QR code as a PNG image. Place this image where your audience can scan it using a QR code scanner, such as on a mobile device, to access your form or quiz.
  • Via email: Upon selecting this option, your email application will open. Add the recipients' email addresses, and note that Microsoft Forms automatically includes a brief note and a link to your form in the email body. Feel free to customize the email message to suit your needs.

Viewing Results

To analyze the results of your form, first open the specific form in question, and then select the Responses tab.

  • Click on View Results to examine individual responder details. This includes the duration they took to complete your form and their chosen responses.
  • For a comprehensive view of all response data in a structured format, select Open in Excel. This action will export the data to an Excel workbook.
  • To delve deeper, select the More Details link. This provides access to each responder's name and their specific answers to each question in your form.
