Faculty are not covered under short term disability benefits; however, they may apply for leave of absences or reductions in load due to medical reasons. Appropriate medical documentation must be provided to process an approved paid and/or unpaid medical leave of absence.

Please note, the Leave of Absence or Reduction in Load Application form must be completed, signed and forwarded to your department chair, dean, and provost for approval. A copy of the approved signed form must then be forwarded to Employee Benefits & Work/Life Department.
