Who is the Audience for this Service?

This service is intended for faculty, staff, students, and form administrators who need help with forms accessed through the ICWorkflow portal.

What is this Service?

Use this service to report problems with forms in IC Workflow, such as technical issues, or to request help with submitting or approving a form. Form administrators may also use this service to request changes to an existing form.

This service is not for requesting new forms. To explore migrating a form to Slate, Formstack, or another form/workflow tool and adding it to the ICWorkflow portal, please submit a request for a Project & Process Innovation Consultation for further evaluation.


The Request ICWorkflow Support service allows faculty, staff, students, and form administrators to report issues, request help, or ask for changes related to forms accessed through the ICWorkflow portal. These forms may be hosted on various platforms such as Slate, Formstack, or others, but are all linked through the ICWorkflow Portal.

Use this service to:

  • Report technical problems or errors with ICWorkflow-linked forms
  • Request assistance with submitting, approving, or routing forms
  • Request updates or modifications to existing forms (Form Administrators only)

Requests submitted through this service will be reviewed and directed to the appropriate support team. Response times may vary depending on the nature of the issue or request.
